Description of activityDiabetes Program consists of two methods of support: Diabetes Education, Diabetes Support Group.Diabetes Education: Primero Health partners with local churches,schools,libraries,and Senior Living facilities to coordinate and deliver the Diabetes Education Empowerment Program (DEEP), which are six-week workshops offered at no cost to participants. Primero Health provides these prevention and self-management workshops, as well as health screenings, in targeted communities where people are underserved and often uninsured. Primero Health completed four DEEP Workshops in 2016.Diabetes Support Group: Primero Health established a Diabetes Support Group that is attended by community members diagnosed with diabetes, as well as friends and/or family members of someone diagnosed with diabetes. This designation allowed Primero Health to assist individuals and families understand, apply, and enroll in health coverage through the federally-facilitated Health Insurance Marketplace during the inaugural enrollment period. The open enrollment period was October 1, 2013 March 31, 2014. Nine (9) volunteers completed federal training to become Certified Application Counselors. Primero Health partnered with local churches,libraries, recreation centers, and schools to provide free Zumba Fitness Classes to residents in underserved communities who have limited access to affordable fitness programs. Classes are ongoing throughout the year at five different locations, and other locations are being added each year. In all locations, child care is provided at no cost to participants at every class. Weight, BMI (body mass index), and waist measurements are taken at the start and end of each session. Class monitoring is conducted every 12 weeks at all locations. Other resources and materials, such as healthy eating handouts, stretch bands, sweat bands, and water bottles are given as incentives as well as healthy fruit/ drinks at the end of each session. This program teaches basic nutrition and emphasizes the selection and preparation of fresh, seasonal foods that are nutritious and economical. Participants meet once a week for six weeks to learn nutritious, healthy cooking habits. Every participant leaves each class with recipes and groceries to practice their new acquired cooking skills. Primero Health has two master trainers.
