Services and supports focus on community living skill acquisition and community participation, with a core belief that all persons are functioning and contributing members of their communities. Programs are provided to adults with an Intellectual and Developmental Disability (IDD) in a variety of settings ranging from 24-hour to intermittent supports. Each resident has an individual, person-centered plan to outline the type and frequency of services and supports they desire and require. The Envision Unlimited Host Family Program allows persons with intellectual and developmental disabilities to live in their community in the private home of a family. The model ensures that every Envision Unlimited client continues to set consistent, important personal and professional goals, with the caring support of Envision staff and their Host Family. Staff visits several times each month to ensure that the clients and host family are effectively dealing with any issues or concerns, and that clients are receiving a high standard of care.Envision Unlimited was chosen by the Illinois Department of Human Services, Division of Developmental Disabilities to implement an innovative new residential service program designed to serve and stabilize individuals with behavioral challenges. Our Short Term Stabilization Homes (SSH) have a combination of intensive staff support and evidence-based treatment in a small group environment, the goal of our SSH's is to prevent hospitalization or placement in a state institution, and to help individuals transition back to a stable living situation in their communities.Envision also provides Home-Based Facilitation to individuals and families receiving funding through the Home-Based and Respite program. Participants in the Home-Based program work with an Envision Qualified Intellectual Disabilities Professional (QIDP) to arrange needed services. The QIDP service facilitator will lead an annual individual service plan and coordinate monthly services and needed resource agreements. Families meet with Envision staff members at least six times per year.
